Integration will require the development of ethics, legal, institutional and regulatory policies for the treatment of patients with psychiatric disabilities, substance abuse problems, physical or mental disabilities, and complex medical conditions. These policy changes are needed to create systems where the interests of patients are taken into account in decision making. We all should care about the needs of our patients; but, in my view, we cannot wait to take care of our patients until integration is complete in the United States. In closing, I would like to offer some personal perspectives that I have gleaned from my research on integrated health care systems. I have had the opportunity to visit several medical institutions in the United States that provide integrated health care systems, and I have met many patients who will never have the chance to live with their loved ones in this country. These are people who are sicker, not just because of medical conditions, but also because of mental illness, substance abuse problems, and other disorders. It is my personal opinion that integrated health care systems do not work.
The integration process is highly fragmented and complex, and the outcomes are far below the expectation of the system. Many patients are not getting the care they need; and many others are having an adverse impact on both their health and well-being. We need to understand the health care system well before any changes are made. As a result, there is great value in developing a set of ethical principles to follow, and that is what this paper is about. First, the term integration refers to the process of transferring an organization's core business functions to a new organization.
