Private health plans are required by law to provide a certain amount of Part A coverage, and many have done so. For many beneficiaries, however, the cost of Part A services alone is a large portion of the cost of Medicare and Medicaid. There are other mechanisms through which private insurance companies are reimbursed for Medicare and Medicaid services, including: 1) the payment of a share of the costs of any drugs that the insurer reimburses, and 2) the payment of a percentage of the amount of Part A services provided to beneficiaries, which is generally the total fee for a full day of service. Private insurance companies also get reimbursed for any drugs or procedures that are medically necessary. Private plans that do not provide these services get a percentage of the cost of any drugs or procedures. Although they are not required under the Medicare and Medicaid regulations to provide services under these arrangements, the fact that they are reimbursed in advance makes it very easy to get patients to purchase these items.
The most recent example of private reimbursement for Part A procedures came with the introduction of private insurance that replaced government-paid Part A services. If an insurer did not offer Medigap plans, or if they did not meet a certain criteria, a taxpayer-funded premium was deducted from the payer's payment to the private health insurer. The amount of the deductible, the portion of the premium the government paid, was calculated in part, but not entirely, based on a person's income. In the 1980s the federal government stepped in and created health care tax subsidies to allow providers to buy care from private companies like HCA for less than the price they paid for it, and to cover more of their costs. These new subsidies are now being phased out under the Affordable Care Act and the federal government will pay for 70 percent of the cost of private insurance for most Americans.
In the future, the new system is likely to bring about a dramatic change of strategy for the large majority of health care providers. For-profit health care enterprises will continue to have a strong incentive to keep patients at home, or at least at the hospital that has the most business. In the past this was an easy choice. For-profit hospitals provided the most expensive and best-trained staff. If they were to be replaced by non-profit, hospital-owned enterprises, those that have the best infrastructure, or the best doctors and hospitals, they would be the ones to hire the most staff and the most expensive. The result will probably be a shift toward a system that is more like that of an urban, non-profit hospital, rather than one that is more like that of a for-profit enterprise. Hospitals in particular will benefit from the new incentives.
The same is true of physician practices and home health agencies. As general health have access to high-quality care from home and in-home nursing, the incentives to stay in the hospital will become greater. This is not to general health that all physicians will shift from being physicians to being home health agents.
